PERMIT <br /> California Street Landfill <br /> 17 LEA Conditions [NOTE LEA conditions listed here shall be in addition to conditions of other documents controlling operation ofthis facility) <br /> 1 The operator shall comply with all State Minimum Standards for Solid Waste Handling and Disposal as specified in Title 14 <br /> California Code of Regulations (CCR) The operator shall not operate this facility without possession of all required <br /> permits/regulatory approvals The operator shall inspect the site at least once each day of operation to ensure compliance <br /> with all applicable standards/conditionsi mitigations/permits/regulations <br /> 2 The operator shall comply with all mitigation and monitoring measures developed in accordance with any certified <br /> environmental document filed pursuant to Public Resources Code, Section,210816, and any administrative/ <br /> enforcement orders of regulatory agencies with jurisdiction at this facility <br /> 3 The operator shall maintain a complete copy of this Solid Waste Facilities Permit (SWFP), of all other required <br /> regulatory permits and of all regulatory inspection reports, at the facility at a location readily accessible to facility <br /> personnel and to the LEA/other regulatory agency personnel <br /> 4 Additional information concerning the design/ operation of this facility shall be furnished upon request to the LEA/ <br /> other regulatory personnel <br /> 5 The operator shall comply with all Notices and Orders issued by any Responsible Agency designated by the Lead <br /> Agency to monitor the mitigation measures contained in any of the documents referenced within this SWFP pursuant <br /> to Public Resources Code, Section 21081 6 <br /> 6 No significant change in design or operation from that described in section 5,Specifications, of this permit is allowed <br /> 7 This facility is authorized to conduct limited salvaging and to store recovered materials (if such salvaging/ storage is <br /> properly described in the RDSI or amendments thereto) for brief periods of time [not to exceed thirty (30) days for any <br /> category of material] and only in closable durable containers or other method as specified by the LEA Such limited <br /> salvaging/storage shall only be conducted as pre-approved by the LEA to preclude the creation of health hazards or public <br /> nuisances The facility shall not to be used as a composting facility, materials recovery facility (MRF), processing facility, <br /> transfer station and/or transformation facility. No recycling is allowed at night without prior LEA written approval No <br /> crushing, baling, grinding, mechanical sorting, composting, or other processing shall occur at the facility location except <br /> as the LEA may give prior written approval <br /> 8 The operator shall notify the LEA, in writing, of any proposed changes in the routine facility operation or changes in <br /> facility design during the planning stages In no case shall the operator undertake any changes unless the operator <br /> first submits to the LEA a notice of said changes at least 150 days before said changes are undertaken Any <br /> significant change as determined by the LEA would require a revision of this permit <br /> 9 This SWFP is subject to review by the LEA and may be suspended, revoked or modified at any time for sufficient <br /> cause <br /> 10 The LEA reserves the right to suspend and/or modify operations at this facility when deemed necessary due to any <br /> emergency, potential health hazard, and/or public nuisance <br /> 11 The operator shall comply with the requirements of all applicable laws pertaining to employee health and safety <br /> including maintaining an up to date written CAL OSHA Injury and Illness Prevention Program (IIPP) on-site at a <br /> location readily available for review by all facility employees and by the LEA/other regulatory agencies. The IIPP shall <br /> include a comprehensive training plan, availability of all necessary on-site safety equipment, and adequate on-site first <br /> aid supplies Whenever personnel are at the site they shall have immediate radio and/or telephone access to an <br /> emergency dispatcher <br /> 12 The operator shall maintain an LEA approved hazardous/ prohibited waste screening/ exclusion (loadchecking) <br /> program which will adequately protect public health and the environment from illegal on-site disposal of hazardous/ <br /> prohibited wastes The operator shall comply with provisions of title 14 CCR,§ 17258 20(Procedures for Excluding the <br /> Receipt of Hazardous Waste) <br /> 16fd7195 Wca5fp4408M369 <br /> 17195mvf01N0 <br />
